NEXUS LETTER REQUEST
Dear Dr. [Physician Name], I am a veteran filing a VA disability compensation claim for [diagnosis] and I am requesting your assistance in preparing a nexus letter. A nexus letter is a medical opinion that establishes the connection between my current diagnosis and my military service. It is often the most important document in a VA disability claim.
THE LEGAL STANDARD THE VA REQUIRES
The VA requires an opinion stating that my condition is at least as likely as not (50 percent or greater probability) connected to my military service. This is the minimum standard. Stronger language such as more likely than not or is caused by carries more weight.
WHAT THE LETTER MUST INCLUDE
- Your full name, credentials, license number, and contact information
- A statement of your familiarity with my medical history
- My current diagnosis using standard medical terminology
- Description of the in-service event or exposure being linked
- A clear opinion using the at-least-as-likely-as-not standard or stronger
- Medical rationale supporting the opinion including record review and clinical findings
- Your signature and date

What is the at-least-as-likely-as-not standard?
This is the legal threshold the VA uses to evaluate nexus opinions. It means the physician believes there is a 50 percent or greater probability that the condition is connected to military service. Your doctor does not need to be certain -- just that it is at least as likely as not true.
Can my primary care physician write a nexus letter?
Yes. Any licensed medical professional who is familiar with your condition and medical history can write a nexus letter. Specialists who treat your specific condition often carry more weight, but a well-documented letter from your primary care physician is valid.
What if my doctor refuses to write the letter?
Some physicians are unfamiliar with VA claims or hesitant to write nexus letters. If your doctor declines, ask for a referral to someone experienced with VA documentation, or contact a VSO who may know providers in your area who regularly assist with nexus letters.
How it works
- Describe your situation. Enter your details in plain English. No document upload required for most VA tools. Your information is used only to generate your output and is never stored.
- We analyze it. Bill Advantage reviews your input against VA regulations, benefit rules, and current 2026 rates, reflecting the knowledge of VA billing and benefits professionals encoded into a system that works in minutes, not days.
- You get answers. Receive a plain-English explanation of exactly what applies to your situation. For most tools, a ready-to-send letter or worksheet is included. Set reminders for follow-up deadlines and build a complete record over time.
